The Transition Support Practitioner supports children and young people during periods of transition, including reunification, placement changes, or transitioning to independent living, ensuring transitions are smooth, practical needs are met, and each child’s emotional and therapeutic needs are supported. The Transition Support Practitioner assists by implementing practical strategies that ensure children and young people are appropriately supported through all stages of their placement, while also ensuring that NDIS plans and supports are aligned with their needs.
This role involves working in partnership with the Therapeutic Specialist, Foster Care Case Manager, carers, families, and external agencies, including National Disability Insurance Scheme (NDIS) providers, to ensure that transitions are smooth, practical needs are met, and each child’s emotional and therapeutic needs are supported.
About you
We are looking for a dynamic and experienced practitioner who can demonstrate:
- At least 3 years of experience in case management, support coordination, or a related role, with a focus on children and young people with complex needs and disabilities.
- A strong understanding of the NDIS system, including the ability to work collaboratively with NDIS providers to develop, review, and implement support plans.
- Experience working in partnership with therapeutic professionals, case managers, and carers to develop and implement cohesive, practical support plans for children and families.
- Tertiary qualifications in Social Work, Disability Services, Psychology, or a related field.
